Where should a toilet roll holder be fitted?

Roughly 20 to 30cm forward of the front of the pan and about 65 to 70cm from the floor, on whichever side you reach naturally. Too far back and you cannot reach it seated.

Do open ended holders drop the roll?

Not in normal use — the arm is angled or lipped to retain it. They are chosen precisely because the roll goes on and off easily without a spindle to compress.
